Digestion: MDCAT Notes and MCQs

Alimentary canal & functions

PartSecretion / function
Mouth (oral cavity)Mastication; 3 pairs of salivary glands (parotid, submandibular, sublingual) โ†’ saliva โ‰ˆ 1.5 L/day with salivary amylase (ptyalin): starch โ†’ maltose; mucus lubricates; lysozyme.
Pharynx & oesophagusSwallowing; food moved by peristalsis; no digestion.
StomachGastric glands: parietal (oxyntic) cells โ†’ HCl (kills microbes, activates pepsinogen, pH โ‰ˆ 2); chief (peptic/zymogen) cells โ†’ pepsinogen โ†’ pepsin (proteins โ†’ peptides); mucous cells โ†’ mucus. Hormone gastrin stimulates gastric juice. Food becomes chyme.
Small intestine (duodenum, jejunum, ileum)Completes digestion and absorbs nutrients; villi & microvilli increase surface area; lacteals absorb fats. Intestinal enzymes: maltase, sucrase, lactase, peptidases, lipase.
Large intestine (caecum, appendix, colon, rectum)Absorbs water and salts; bacteria make vitamin K; forms and stores faeces.

Associated glands

  • Liver (largest gland): produces bile (stored in gall bladder). Bile has no enzymes; bile salts emulsify fats and neutralize acid chyme. Liver also stores glycogen, deaminates amino acids (urea), detoxifies, makes plasma proteins.
  • Pancreas (mixed gland): pancreatic juice (alkaline, HCO3โˆ’) with trypsin (secreted as trypsinogen, activated by enterokinase), pancreatic amylase, lipase (fats โ†’ fatty acids + glycerol), nucleases. Islets of Langerhans โ†’ insulin, glucagon.

Hormonal control

Gastrin (stomach) โ†’ gastric secretion. Secretin (duodenum) โ†’ pancreatic bicarbonate (first hormone discovered โ€” Bayliss & Starling, 1902). Cholecystokinin (CCK) โ†’ gall bladder contraction & pancreatic enzymes.

PMDC learning outcomes for this unit
  • Human digestive system: 14.1 Describe the parts of human digestive system
  • Human digestive system: 14.2 Explain the functions of the main parts of the digestive system including associated structures and glands

Digestion MCQs with answers

Practise and track your score โ†’
Q1
Salivary amylase converts starch into:
ASucrose
BMaltose
CLactose
DGlucose
Show answer
Answer: B (Maltose). Ptyalin breaks starch into maltose (a disaccharide).
Q2
HCl in the stomach is secreted by:
AParietal (oxyntic) cells
BGoblet cells
CChief cells
DMucous cells
Show answer
Answer: A (Parietal (oxyntic) cells). Parietal cells secrete HCl; chief cells secrete pepsinogen.
Q3
Pepsinogen is converted into active pepsin by:
AHCl
BTrypsin
CEnterokinase
DBile
Show answer
Answer: A (HCl). HCl (and pepsin itself) activates pepsinogen.
Q4
Bile helps in digestion of fats by:
AEmulsifying them
BConverting them to glucose
CHydrolysing them
DAbsorbing them
Show answer
Answer: A (Emulsifying them). Bile salts break fat into small droplets, increasing surface area for lipase; bile has no enzymes.
Q5
Trypsinogen is activated in the duodenum by:
AEnterokinase
BPepsin
CBile salts
DHCl
Show answer
Answer: A (Enterokinase). Enterokinase from the intestinal wall converts trypsinogen into trypsin.
Q6
Most absorption of digested food occurs in the:
AStomach
BSmall intestine
COesophagus
DLarge intestine
Show answer
Answer: B (Small intestine). Villi and microvilli of the small intestine provide a huge absorptive surface.
Q7
The main function of the large intestine is absorption of:
AGlucose
BAmino acids
CWater and salts
DFatty acids
Show answer
Answer: C (Water and salts). The colon absorbs water and electrolytes, forming faeces.
Q8
The hormone that stimulates release of bicarbonate-rich pancreatic juice is:
AGastrin
BSecretin
CGlucagon
DInsulin
Show answer
Answer: B (Secretin). Secretin from the duodenum stimulates the pancreas to secrete HCO3โˆ’.
Q9
The largest gland in the human body is the:
AThyroid
BSalivary gland
CLiver
DPancreas
Show answer
Answer: C (Liver). The liver is the largest gland.
